Referrals

Referrals to D.L.C.R.S may be made by contacting the main office at 416-281-1147 or via e-mail at info@dlcrs.com . Please ask to speak with the Intake & Family Facilitator and they will assist you in the referral process.  A preliminary decision with respect to space and suitability to the agency will be given immediately.  If the decision is to proceed with the referral, written material outlining the child/youth's strengths, challenges, current abilities, current school reports, and any assessments will be requested.  Once the documentation has been reviewed, an opportunity to meet the child/youth in an environment they are comfortable in such as home or school will be arranged in an appropriate time frame.

If after review of the documentation and meeting with the child/youth, it is determined that they would be a good match, the family, legal guardian, and the referring agency will be notified.  The next step will be for the family members, legal guardian(s), and Case Managers to visit the residence, meet the staff, and other children/youth and ask questions prior to a final decision being made.

If it is determined that placement for the child/youth will proceed, D.L.C.R.S. will discuss with the family/legal guardian(s), and Case Manager, the most suitable transition plan to aid the child/youth in transitioning to their new residence.  Pre-admission visits will be scheduled but it will be determined on a case-by-case basis how many pre-admission visits, duration, and if overnight visits would be in the child/youth's best interests.

Prior to admission, all children/youth must have a pre-admission medical, updated immunizations, a report from the last dental visit and complete any COVID-19 protocols that are in place at the given time of admission.

There are times when D.L.C.R.S. is able to accept emergency referrals where the opportunity for the above process may not be feasible and a decision is needed at that time.
